BLUE SLIP REQUIREMENTS - PROOF OF INFORMATION
If you are 15 to 17 years old, you can attend the Driver's Education program. However, restrictions allow only "eligible" students to received their Certificate of Completion ("Blue Slip"). State Eligibility Requirements for Drive Home Safe to submit for your Blue Slip are as follows.
If you are currently enrolled in a high school (public or private school):
- You must verify that you have had at least eight (8) passing grades during the previous two (2) semesters last ending prior to requesting a Blue Slip from the Secretary of State.
- Proof of this must be documented using one of the following:
- Report cards
- Transcripts
- Driver Education Approval Form (signed by your school)
If you are enrolled in high school but have not passed eight (8) courses during the previous two (2) semesters:
- Proof must be documented by the Driver Education Approval Form (signed by your school)
If you are dropped out of school - several options apply:
- If you have received eight (8) passing grades in the two (2) semesters prior to ordering your Blue Slip, proof of this must be documented by using the following:
- Report cards
- Transcripts
- Driver Education Approval Form (signed by your school)
- Provide a copy of your G. E. D. course of enrollment in an Alternative Education Program
- Obtain written consent of your parent(s)/guardian and your regional superintendent
- Obtain a waiver from the district superintendent or chief school administrator from the school district in which you reside.
If you are Home Schooled:
- Parental Consent Form signed by parent or guardian
Note: We can not order the Blue Slip without one of these up to date verifications. In addition, you must also meet all of the requirements of the course.
